Complete your Health Forms
You will need to complete the following forms and submit them to Health Services by July 31:
- Health Record Form
- Physical Examination Form – please make sure to include your Immunization Record (if your Immunization Record is unavailable, please put that info in writing and submit it to Health Services.)
- For Resident Students Only – Meningitis Waiver Form & Info on the Meningococcal Vaccines
