Complete your Health Forms
You will need to complete the following forms and submit them to Health Services before you arrive on January 13:
- Health Record Form
- Physical Examination Form – please make sure to include your Immunization Record (if your Immunization Record is unavailable, please put that info in writing and submit it to Health Services.)
- For Resident Students Only – Meningitis Waiver Form & Info on the Meningococcal Vaccines
All forms should be mailed or faxed to (all forms should be sent by Mon, January 7; if forms are completed after January 7, bring completed forms with you to Registration on Saturday, January 13):
