If you need to use one of these forms, please be sure to submit it to the LBC Financial Aid Office. These do NOT get sent to the Department of Education!
- Appeal Loss of Financial Aid - for students who have lost financial aid eligibility due to academic progress.
- Church Matching Scholarship
- Graduate School Scholarship Application - for Graduate School students. Deadline for Fall is August 1, 2008. Deadline for Winterim and Spring is December 1, 2008. Deadline for Summer is April 15, 2009.
- International Student Financial Aid Application - All International Students must complete this form in order to apply for scholarship assistance.
- Request Special Circumstances Form - Dependent Students - use this form if your family recently had a major reduction in income, or if there are unusually excessive medical bills, or if your parents are paying private school tuition for one or more of your siblings while you are in college.
- Request Special Circumstances Form - Independent Students - use this form if you or your spouse recently had a major reduction in income, or if there are unusually excessive medical bills, or if you are paying private school tuition for your child(ren).
- Verification Worksheet - use this form if you received a notice from the Financial Aid Office that your FAFSA was selected for Verification.
